Patient Waiver and Responsibility Agreement

Prime Life Diagnostics Ltd (Company Number: 16601360), a private limited company registered in England, with its registered office at 9 Inglass Drive, Edlington, Doncaster, England, DN12 1DG (referred to as “Prime Life Diagnostics,” “we,” “us,” or “our”).

The Patient (referred to as “the Patient,” “you,” or “your”).

1. Purpose of This Agreement

This Agreement sets out the terms and conditions under which Prime Life Diagnostics provides private blood testing services to you. It clarifies the scope of our services and your responsibilities regarding your health and the interpretation of your test results. By proceeding with your booking, you acknowledge that you have read, understood, and agree to be bound by its terms.

2. Nature of Services Provided by Prime Life Diagnostics

Prime Life Diagnostics provides private blood testing services, which include:

  • Collection of blood samples (at our facility or through a partner phlebotomy service).
  • Processing of samples through accredited laboratories.
  • Delivery of laboratory test results to you.

Prime Life Diagnostics does not provide:

  • Medical diagnosis of any health condition.
  • Medical advice, consultation, or clinical interpretation of test results beyond the laboratory data.
  • Treatment recommendations for any health condition.
  • Emergency medical services.
  • A substitute for ongoing care from your own qualified healthcare professional.

3. Your Acknowledgements and Responsibilities

  1. Existing Healthcare Provider: You confirm that you have an ongoing relationship with a qualified healthcare professional (such as a GP or specialist) who is responsible for your overall care.
  2. Responsibility for Interpretation and Follow-up: You are solely responsible for sharing your results with your healthcare professional, seeking advice, and following up on abnormal or concerning results.
  3. No Doctor-Patient Relationship: Engaging with Prime Life Diagnostics does not establish a doctor-patient relationship between you and Prime Life Diagnostics or its staff.
  4. Accuracy of Information: You confirm that all personal and medical information you provide is accurate and complete.
  5. Understanding Limitations: You acknowledge that blood tests are one component of health assessment and should not be used for self-diagnosis or treatment.

4. Disclaimer of Liability

To the fullest extent permitted by law, Prime Life Diagnostics disclaims all liability for any loss, damage, injury, or adverse health outcome arising from:

  • Failure to share results with a qualified healthcare professional.
  • Misinterpretation of your results.
  • Reliance on information from Prime Life Diagnostics as a substitute for professional medical advice.
  • Decisions you make based on your results without guidance from a qualified clinician.
  • Pre-existing conditions or undisclosed health information affecting your results.

This Agreement does not exclude our liability for death or personal injury resulting from our negligence or fraud, or any other liability which cannot be excluded by law.

5. Data Protection and Confidentiality

Prime Life Diagnostics is committed to protecting your privacy and personal data. All information will be handled in accordance with the UK GDPR and applicable laws.

6. Governing Law and Jurisdiction

This Agreement shall be governed by and construed in accordance with the law of England and Wales. The courts of England and Wales shall have exclusive jurisdiction over any disputes arising under it.

7. Entire Agreement

This Agreement constitutes the entire understanding between you and Prime Life Diagnostics and supersedes all prior communications, whether written or oral.

By proceeding with your booking, you agree to all terms set forth in this Patient Waiver and Responsibility Agreement.
